Missouri Discharge Planning Survey about Hospitalized Older Adults
Discharge planning is an important topic, and we hope our research will lead to improvements in the discharge planning process, inform future research, reimbursement strategies, and policy changes to improve discharge planning.
To help accomplish this goal, we invite you to participate in a survey about the challenges health care professionals face when discharging hospitalized older patients.
This survey is in cooperation with the Missouri Hospital Association and the Missouri Chapter of the American College of Physicians; and the investigators are Lori Popejoy, PhD, RN; Kyle Moylan, M.D.; and Colleen Galambos, PhD, MSW.
